E-Sign Terms & Conditions
Consent to Electronic Communications (“Consent”)
You understand that, by electronically signing this agreement, you are confirming that you: (1) have agreed to the terms and conditions of this Consent; (2) have downloaded or printed a copy of this Consent for your records; (3) can access and view electronic documents in the format provided; and (4) have the necessary hardware and software to receive and retain electronic documents. We will maintain a record of your consent for compliance purposes. You agree that:
- Any and all communications concerning any Payment Plan, any disputes, any verification of debt and/or requests for original creditor information, our privacy policy, the status and history of your debt(s), the status and history of your account, and any further disclosures required by federal or state law (the “Disclosures”) may be provided through this website or by email. The Disclosures may include disclosures pursuant to: (1) the federal Fair Credit Reporting Act and Regulation V; (2) the federal Fair Debt Collections Practices Act and Regulation F; (3) the federal Truth in Lending Act and Regulation Z; (4) the federal Electronic Funds Transfer Act and Regulation E; (5) the federal Gramm-Leach-Bliley Act; (6) the federal Telephone Consumer Protection Act; and (7) any other applicable federal, state or local law or regulation.
- Your Consent applies not only to any current payment plan and associated debt(s), but also to any other ancillary agreement(s), or future payment plans or debt(s).
- To electronically receive, access, view, and save, print, or otherwise retain the Disclosures, you will need the following hardware and software:
- A device capable of receiving, accessing, and retaining electronic records with internet access;
- A widely-used, recent-generation web browser (e.g., Chrome, Firefox, or Safari);
- A widely-used, recent generation portable document file (“PDF”) reader or similar software to view documents in PDF format;
- An email address; and
- Either a printer, hard drive, or other storage device.
- You have the right to receive any required document associated with this payment agreement in a non-electronic form.You have the right to withdraw your consent at any time. You may contact us by mail at 175 W. Jackson Blvd., Suite 600, Chicago, IL 60604 or phone at (888) 429-4670 to: (1) obtain a paper copy of any communication at no charge; (2) withdraw your Consent to receive information electronically; or (3) change the email address at which you will receive communications from us. There are no conditions, consequences, or fees associated with withdrawing your consent other than that we would no longer communicate with you electronically.
- If there are changes to the hardware or software requirements necessary to access or retain electronic documents, we will provide you with updated information. If the changes impact your ability to access documents, we will obtain your affirmative consent again, following the procedures outlined above.
